- Makeup, Hair & Styling Agencies that are listed in any of the following directories: LA411, NY411, NYPG, Workbook, LeBook, Alternative Book, Select, The Hair Makeup & Styling Career Guide.

Acceptable forms of ID include:
  1. Copy of a Student ID card with number (Must provide phone number of the school for verification)


    2. OR

  2. Copy of Promo or Comp Card


  3. Name of Agency of Record (Must provide name and phone number of agency for verification)

  4. Resume (Must include phone number of at least 2 references)


    5. OR

  5. I.A.T.S.E. or N.A.B.E.T. Union ID Card number


  6. Costume Designers Guild Card Number
